Using the data from the hypothetical clinical situation that we present to you in this task, carry out your first Nursing Care Plan for this client. You will find the necessary materials and the table to develop the care plan in the Supplementary Materials section Model 2:

  1. Identify in order of priority two (2) heath needs of this hypothetical client, one(1) real one and one (1) at risk, that are the highest priorities when offering nursing care in this hypothetical situation.
  2. It lists the objective and subjective data that evidences the existence of the need for health.
  3. Identify the focal stimuli related to both selected needs.
  4. Using the NANDA 2018-20 labels and the PES formart, write each of the two priotity nursing diagnoses, real and st risk.
  5. Write one (1) expected result for each nursing diagnosis identified in this situation.(Use the format: subject, action verb, maladaptive behavior modification indicator/ res and time to achieve it) as learned in modul 2.
  6. Putting in order of priority, write at least five (5) nursing interventions that you would carry out to achieve each of the expected results. It is important to include in the interventions, at least one for prevention and promotion of client-family health.
  7. Mention the Scientific rationale that validates carrying out each one by intervention
  8. Write an assessment for each nursing diagnosis in light of the expected outcome you developed.

Hypothetical Clinical situation:

A 75-year old man is admitted to the Emergency Room with a picture of acute respiratory faiure compatible with an infection by the COVID-19 virus. The client’s vital signs are: blood pressure 80/55 mmHg, heart rate 120 beats per minute, 34 breaths per minute, temperature 38.7 degree centigrade, and Sat O2 85%. Client has a history of pre-existing medical conditions: high blood pressure, diabetes mellitus, obesity, and bronchial asthma. The client smokes about 30 cigarettes a day and drinks alcohol with meals and on weekends. The medical diagnosis is of a suspected pneumonia secondary to COVID-19 infection. The doctor on duty prescribes to conduct laboratory tests for the Sars-Cov-2 virus and various other hematologies and chemicals; initiate intravenous fluid therapy, monitoring fluid intake and discharge every 2 hours, and insertion of a urinary catheter; reservoir masks oxygen therapy, bronchodilator inhalers, and broad-spectrum antibiotic therapy.
